How to Register
If you wish to register as a patient please ask at reception for a new patient pack. You will find two forms in the pack. Both forms must be completed. Once the forms are completed, please return them to reception. As part of our service to you all new patients are offered a new patient health check. Please book this with reception when you return your forms.
Once registered, we can request your medical records from your previous GP via the Health Authority. Unfortunately we cannot accept everyone who requests registration. We have a boundary within which we work and only those living in that area can be accepted to, or can remain on, the patient list. This boundary is overlapped by other surgeries so there is always the option of more than one practice to register with.
Please remember to notify the practice if you change name, address or telephone number. Please put any amendments in writing by fax, post, or you can come to the surgery to complete a form. Alternatively, click here to send amendments via this site.
Appointments
Appointments can be made in person at our reception or by phone during opening hours. We endeavour to offer an appointment within 24 hours with a health care professional (Doctor or Nurse) or 48hrs with a GP. Appointments for a named GP may take a little longer. Our reception staff may need to ask for details about your problem, in order that an appointment may be arranged with an appropriate clinician. This information will be treated with the utmost confidentiality.
All appointments are for one issue for one person only.
If you are unable to attend an appointment please contact the surgery as soon as possible so that we can offer your appointment to someone else.

Repeat Prescriptions
- A repeat prescription means you can get more of your medication without seeing a doctor
- Please ask for a repeat when you have about a week’s supply left
- To make a request
- Tick the box to the left hand side of each item you wish to request
- Hand in, send to us or fax the tear off slip from the right hand side of your prescription
- We cannot accept requests by telephone.
- If you would like us to send your prescription to you please include an SAE
- Allow a minimum of 2 full working days for processing
- Usually prescriptions are for a maximum of 2 months supply (at your doctors discretion)
- They are reviewed at least every 6 months
- You may be asked to make an appointment, or have a blood test as a result of this review
Home Visits
If a home visit is required, please telephone 020 8299 0499 before 10.30am. Only if you are too ill to attend the practice, or housebound by chronic disease, will a doctor visit you at home. Visits are normally made after morning surgery. When requesting a home visit please give the receptionist as much information as possible to enable the doctor to allocate priority to house calls.
You may request a telephone consultation if you wish to speak to a Doctor or Nurse for advice. Please ring before 10.30am. If no one is immediately available your call will be returned.
